In Multibilling screen you can download this same account statement but for the whole encounter, not only for one single extension. This can be performed in Encounter Balance (context menu) :

If you want to generate it for only one extension, for example the extension where the insurer or patient charges are, select Balance, this will take you to the Account Statement window, where you can generate it for the selected extension only:
