To add Patient's Lifestyle data, click on this option from Nurse Dashboard :
Lifestyle Parameters can be any of the following :
Current every day smoker
Current some day smoker
Former smoker
Never smoker
Smoker, current status unknown
Unknown if ever smoked
To add new parameters, select it from the list or click on add icon.
2. Now, enter lifestyle information :
Parameter of Lifestyle
Notes
Initial Date of Monitoring
Final Date of Monitoring
Monitoring comments
3. Save.
Parameters will now be saved. To edit or delete this record, double click on it.
You can edit these values or delete the whole record (bin icon) :
