Fill up the Occurrence Variance Report (OVR) for any patient incidents

In OVR screen, you can fill up and view the questionnaire for the Occurrence Variance Report (OVR) for any patient incidents that occurred during ther stay. This questionnaire can be filled up and attach to a patient file of be filled up by a hospital employee or it can even be left as anonymous.


To fill up this questionnaire, follow these steps:

Step 1. Go to OVR window.

Step 2. Click on New Record.

Step 3. Fill Occurrence Variance Report Sections.

Here, you will find four different sections that must be filled up :

A. For Event Date, Location and Description of the incident.

B. For selecting data of the person who filled this up. Also for entering the Approval Request user.

C. The Occurrence Variance Report itself (see image from below).

D. Left column tabs, which will appear after completing steps A, B, and C.

If you select Patient, enter patient name and check the Attach to the patient file if you want to include this OVR Report to their file.

If you selected Employee, enter their username and select their role :

If you want to leave it as anonymous, you will only require to enter the Approve Request user :

Occurrence Variance Report section has 16 different categories for any issues during patient treatment, at the end of this section you will see another section Immediate Action, which is mandatory. See full image at the end of this article.

Now, let's view all of the included sections that can be filled up in OVR. Once you check any issue, any related tabs to these issues will load in the left column.

Patient Safety

Here you can enter patient safety issues such as Patient ID errors, Communication errors, Medication Errors and other patient safety related issues.

Patient Rights

The second section includes options for Patient Rgiths, Content issues and any other patient rights issues.

Clinical / Medical Care

Third section includes options for any medical care issues such as staff availability, transfusion issues, skin related issues, tubes related issues, unexpected outcomes during medical care and more medical care related issues.

Diagnostic Services, OR

Section four is for any diagnostic services issues.

Section five is for any OR issues such as delayed surgeries or canceled surgeries.

Anesthesia

In the next section you can include any anesthesia (intubation or extubation, anesthesia and unexpected outcomes) issues.

Surgical & Infant/Maternal

Section 7 includes options for entering any surgical issues.

Section 8, for Infant/Maternal includes fields for any infant or maternal issues and complications.

Admission/Discharge, Documentation/Entry, Staff related

The following three sections include the following options :

Admission/Discharge : Admission, discharge or readmission issues

Documentation/Entry : Documentation issues (files, entries, document filling)

Staff realated : Staff related issues

Equipment, Safety Concerns

Section 12 includes options for any equipment concern such as defects, malfunctions, equipment unavailability or alarm issues.

Section 13 includes options for safety concerns ; infections, hazmat exposures, fire, false alarms, and facility problems.

Security Concerns, Others

Section 14, includes options for Security Concerns such as vandalism, assault, narcotic issues and any other security issues.

Section 15 includes a section for any other concerns that do not appear in the listed options.

Sentinel Events

Incident Outcomes

Section 16 includes includes an option with the possible outcomes for the concerns given.

At the end of this section you will see a field called Immediate Action, which is mandatory and allows users to give details about the response.

Step 4. Save your changes after completing this tab.

Once you have completed this tab, save your changes in save button :

Step 5. Complete tabs data.

Now, after selecting the options that apply to this OVR document, go to the following tabs that unlocked, they will unlock according to the given issue.

In our given example, we have filled up several tabs for Unavailable Medication issues.

After completing the questions for each tab, save changes :

Step 6. Complete Document.

Now, after filling up every tab (they will be highlighted in green after successfully saving changes), click on gear icon to Process this document and set it as complete :

Step 7. Document Approval.

This document must be reviewed and approved by authorized users, this can be done in Document Approvals from Activies section (Home menu). Click on it to see any pending documents :

This OVR completed document must appear listed here, fill these sections to send the approval message :

A. Forward to. Which is the user to which this approval will be sent via email.

B. Text Message. Which will be the attached message for this approval response.

C. Answer. Select Yes or No, which is the approval response.

D. Confirm.

Steps 6-7 demo :

You can go back to this OVR document in the future to review it and even attach it to a patient record (Attached Documents from Electronic Patient Record).
