1.- First go to Form - Section Configuration and to the main tab. Here is where you can assign medical forms to an organization :

2.- Now go to Graphic menu detail tab from this same window : Here is where you can assign this medical form to any dashboard (Physician Dashboard, Nurse Dashboard, User Dashboard).

3.- To relate this form to any role, go to Form section access tab. Create new record and select role to assign.
