All you need to do is

1- Go to the "Vital sings formula" option

2- Click on the "New record" button

3- Fill all the requiered data and click on the "Save" button

If you want to Edit any of the exiting record you need to select one and click the edit button

After that just modify the data and click on the "Save" button.
