Pharmacists can review and edit prescriptions by marking them as Reviewed and selecting a Reason for editing (A). Next to the reason field you can give more details about this by entering notes in Notes/Suggestion (B) field. The second field can give more details about the reason chosen (not supplied, wrong frequency, discontinued, etc.) :

Nonetheless, these notes were not appearing on Electronic Patient Record (we were only showing discontinued/edited dates) but now users can see this data and review more details about why some medications were edited or discontinued. Reason (A) and Comments/Notes (B) will appear as follows in EPR*:

* Motive Information Route : Electronic Patient Record > Encounter Data tab > Medications subtab > Context menu > More information button (popup).
